BrightSpring Health Services Corporation’s Rehab Without Walls program seeks an experienced Physical Therapist to deliver functional rehabilitation in home and community settings, including home, school, work, and community environments. The role emphasizes autonomous practice within a collaborative therapy team.

The candidate will conduct evaluations, develop and adjust treatment plans, and support patients in real-life activities while meeting licensing and safety requirements.

Qualifications

• Minimum of bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Ability to meet physical requirements listed in the job description.

Responsabilités

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness of treatment plans and participate in staff meetings.
• Conduct discharge evaluations and provide home/community safety recommendations.
• Communicate patient needs and progress to the treatment team and family.
• Coordinate with other disciplines to optimize patient outcomes.
