Job summary

Rehab Without Walls Neuro Rehabilitation is seeking a licensed Physical Therapist to deliver home and community-based rehabilitation. You will assess, plan and adjust treatments in real-life environments, collaborating with an interdisciplinary team to promote independence and functional goals.

The role emphasizes autonomy, client-centered care, and ongoing education to support patients in home, school, work, and community settings.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• Current Basic Life Support or CPR certification in accordance with state regulations.
• Minimum of one year’s work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.
• Ability to lift 50 pounds; up to 100 pounds with safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make recommendations for home and community safety, and provide resources.
• Communicate patient’s needs and progress to the treatment team, physician, patient and family.
• Collaborates with other disciplines to enhance patient outcomes.
