Rehab Without Walls Neuro Rehabilitation is seeking an experienced Physical Therapist to deliver real-world, functional rehabilitation in home and community settings. You will design individualized plans, reassess progress, and coordinate with an interdisciplinary team to improve patient outcomes.

Qualifications
- Active PT license in practicing state.
- Degree in Physical Therapy from an accredited program.
- BLS/CPR certification as per state regulations.
- At least one year of physical therapy experience preferred.
- Knowledge of rehabilitation techniques for neurological injuries.
- Ability to lift 50 pounds and move up to 100 pounds.
- Strong stamina for prolonged standing and walking.
Responsibilities
- Perform patient evaluations and develop individualized treatment plans.
- Regularly reassess treatment effectiveness; participate in staffing and family conferences.
- Conduct discharge evaluations and provide home/community safety recommendations.
- Communicate patient needs and progress to the interdisciplinary team, physicians, patients, and families.
- Collaborate across disciplines to coordinate care and improve outcomes.
