Job summary

Elara Caring is seeking a Care Team Supervisor for Personal Care Services to lead a team of caregivers on-site in a fast-paced home care setting. The role focuses on ensuring accurate records, plans of care, and timely service delivery while supporting branch growth initiatives.

The position requires supervisory experience, strong organizational and communication skills, and a willingness to travel locally for audits or operational needs.

Qualifications

• High School Diploma or GED required.
• 60 college credit hours required and/or alternative qualifications accepted.
• Two years of supervisory experience required.
• Associate’s or Bachelor’s degree preferred.
• Experience with elderly or disabled populations preferred.

Responsibilities

• Maintain client service records, plans of care, caregiver schedules, and documentation at the branch.
• Review and manage service authorizations and contract-specific requirements.
• Support branch growth initiatives and operational planning.
• Ensure compliance with licensure, payer, contract, and regulatory standards.
• Provide guidance, coaching, and support to caregivers to promote performance.
