Addus HomeCare in Arcadia/San Rafael is seeking a driven Service Coordinator to lead scheduling of in-home care workers and clients across a geographic area. You’ll be the go-to problem solver—juggling schedules, supporting field staff, handling client updates, and ensuring care plans stay on track.

Full-time hours with on-call rotation every other weekend. You’ll supervise, conduct in-home visits, and help maintain high-quality care for aging adults and individuals with disabilities.

Qualifications

• 3 to 5 years of industry experience required.
• Must have high school diploma or equivalent.
• Interpersonal, organizational and communication skills required.
• Computer skills including Microsoft Word, Microsoft Excel and Scheduling program.

Responsibilities

• Schedule and supervise in-home care workers and clients in a geographic area.
• Provide supervision and support to field staff and ensure care plans are followed.
• Conduct in-home visits and respond to client updates and escalations.
• Maintain confidentiality and adhere to regulations and company policies.

We’re looking for a driven, organized, and compassionate Service Coordinator to lead the charge in scheduling caregivers, ensuring top-quality service, and conducting in-home visits. You’ll be the go‑to problem solver—juggling schedules, supporting field staff, handling client updates, and stepping in to keep care plans on track. Hours: Full Time (Monday through Friday On call rotation every other weekend (Saturday, Sunday and holidays)

Pay: $25 to $30 hourly

Position Summary: Responsible for scheduling and supervising in-home care workers and clients in a geographic area. • Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence.
• Provides alternate coverage to ensure the client’s care plan is followed and client services are not interrupted.
• Contacts care providers and clients to provide service updates
• Conducts monthly client wellness calls and conducts home visits as required
• Provides thorough, complete follow-through on escalated client complaints and theft claims
• Supervises direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company’s policies and guidelines
• Ensures the appearance of the branch’s open environment is professional: neat, clean, orderly and generally free of clutter
• Maintains a high degree of confidentiality at all times due to access to sensitive information
• Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the agency
• Follows all MCO, Medicare, Medicaid, and HIPAA regulations and requirements
• Abides by all regulations, policies, procedures and standards

• Must have high school diploma or equivalent.
