Responsibilities

• Coordinate scheduling of home care assignments and adjust schedules as needed.
• Ensure required authorizations are in the EMR for approved care.
• Collect Care Logs/Timesheets from vendors and in-home staff.
• Provide reminders and materials to promote a smooth experience for providers and participants.
• Maintain current records of all scheduled assignments and outcomes.
• Be flexible with schedules, including evenings, nights, weekends, and overtime.
• Complete all required documentation accurately and on time.
• Protect privacy and confidentiality of company information and participants.
• Follow policies, participate in trainings, and communicate workload and priorities.
• Complete mandated training and health screenings timely.
• Adapt assignments as business needs evolve.

Job Summary

The individual in this role is responsible for coordinating the scheduling and tracking of home care and home care assistant assignments. This position requires providing outstanding customer service to all stakeholders, including participants, families, vendors, and staff. The role involves maintaining current and accurate records of all scheduled appointments and collaborating with colleagues and stakeholders to promote organizational values, team, and culture.

Job Responsibilities

• Effectively coordinate the scheduling of home care personal care assignments, including adjusting schedules based on changes and communicating with all stakeholders such as participants, family, and home services staff.
• Ensure required authorizations in the electronic medical record (EMR) for approved personal care.
• Ensure all Care Logs/Timesheets are received from vendors and in-home staff visits.
• Implement procedures to ensure a successful experience for providers, staff, and participants, such as reminder calls, instructions, or sending materials ahead of time.
• Maintain current and accurate records of all scheduled home care personal care assignments, including both the scheduled times and outcomes.
• Be willing and able to work a varied schedule, including evenings, nights, weekends, and overtime.
• Complete all required documentation in a timely and accurate manner.
• Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
• Follow company policies and procedures and participate in any required quality improvement activities, staff training, and meetings.
• Communicate regularly with supervisors, coordinators, and staff regarding workload and priorities.
• Timely completion of all mandated training and education.
• Timely completion of all mandated occupational health screenings as needed.
• Exercise flexibility in performing assignments as business needs evolve.
