Job summary

Community Health Network, Inc. is seeking an experienced Home Care Registered Nurse (RN) for our Danvers, MA coverage area. You will manage a caseload in home care, hospice, and case management, from admission to discharge, and supervise LPNs and Home Health Aides.

We offer flexible options, competitive pay, and mileage reimbursement. A minimum of 1 year of Home Care RN case management experience performing OASIS visits with a Medicare-certified agency is required.

Qualifications

• Minimum 1 year of Home Care RN case management experience.
• Experience performing OASIS visits with a Medicare-certified agency.
• Strong patient and family communication skills.

Responsibilities

• Manage a caseload of home care patients from admission to discharge.
• Supervise LPNs and Home Health Aides.

Andover, MA areas. The Home Care Registered Nurse is responsible for effectively managing and caring for a caseload of patients and their families in the community, from admission to discharge. The Registered Nurse supervises LPN's and Home Health Aides.
