Job summary

Prairie River Home Care, Inc. is seeking an Intake Coordinator to serve as a liaison with referral sources, coordinating referrals and arranging for appropriate home care services. The role supports uninterrupted medical care at home and reports to the Area Manager.

The position requires healthcare experience, knowledge of Medicare and state regulations, strong communication, and organizational skills. Office-based schedule is Monday–Friday, 8 am–5 pm, with comprehensive benefits.

Qualifications

• Minimum of three years health care experience; intake and home health experience.
• Knowledge of Medicare, private insurance and Minnesota Medical Assistance regulations.
• Excellent written and oral communication skills.
• Strong organizational skills and the ability to work independently with minimal supervision.
• Proficient with computers; knowledge of Word and Excel a plus.

Responsibilities

• Receives and coordinates all referrals, including possible notification of insurance providers for prior authorization and services.
• Assists prospective clients and referral sources in identifying appropriate home care services.
• Provides referral source with Company scope of service and coverage area.
• Maintains knowledge and education to remain current as a home health resource to referral sources.
• Maintains confidentiality of all information pertaining to clients, families and employees.
