Live Well Homecare is seeking an Intake Coordinator to process home care referrals with accuracy and efficiency, ensuring policy compliance. You will verify insurance benefits, obtain pre-authorizations, and coordinate discharge planning to support smooth patient transitions.

The role requires multitasking, excellent communication, and strong organizational skills as you liaise across product lines and sites to promote patient satisfaction and timely care delivery.

Qualifications

• High School Diploma or GED required; Associate's Degree preferred.
• 2-5 years of experience, with 5-10 years preferred; experience in home care is a plus.
• Ability to work in a safety-sensitive environment with alertness and safety awareness.
• Strong multitasking, communication, and organizational skills.

Responsibilities

• Screen referrals to confirm patient eligibility and adherence to admission criteria.
• Verify insurance coverage, obtain necessary pre-authorizations, and interpret company policies.
• Track patient status during hospitalization or rehab stays.
• Coordinate discharge planning and educate stakeholders on home care services.
• Assist in pre-scheduling and ensure patients receive appropriate post-acute care.
