Job summary

Columbia River Home Health is seeking an Intake Support Specialist to connect patients with the care they need. The role rewards reliability, teamwork, and strong customer service and computer skills.

You’ll coordinate and process incoming home health referrals, verify patient and insurance information, and enter referral data into the EMR. You’ll communicate with hospitals, physicians, facilities, and case managers to schedule start-of-care dates and keep referrals moving.

Qualifications

• High school diploma or equivalent.
• Medical terminology coursework or equivalent education/experience.
• Medical office experience required.
• Nursing background welcomed but not required.

Responsibilities

• Coordinate and process incoming home health referrals
• Gather and verify patient, insurance, and referral information
• Enter accurate referral information into the EMR
• Communicate with hospitals, physicians, facilities, insurance case managers, patients, and team members
• Help coordinate appropriate start-of-care dates
• Follow up on incomplete referrals and help keep the intake process moving
• Manage incoming fax and website communications
