Job summary

Johns Hopkins Medicine is seeking a Home Care Coordinator Assistant in Baltimore to support the Johns Hopkins Intrastaff home care program. You will assist with coordinating services, verifying insurance, reviewing referrals, and communicating with agencies to initiate care and ensure timely patient discharge.

The role requires 1–2 years of data entry and customer service experience in healthcare, strong interpersonal and organizational skills, and the ability to travel between hospital

Qualifications

• High School Diploma or GED required.
• 1–2 years data entry and customer service experience in healthcare.
• Excellent interpersonal and organizational skills; strong written and verbal communication.

Responsibilities

• Assist the Home Care Coordinator with all related functions.
• Verify insurance and provide information to agency and patients.
• Review referral packets to ensure all required information is present.
• Communicate with agency to initiate care and confirm receipt of referrals.
• Ensure patient discharge occurs or follow up on delays; notify team as needed.
• Maintain relationships with unit staff and outside agencies.
• Input HCC information into the flow sheet; assist with Homecare 101 scheduling.
• File logs and clinical documentation; complete time studies timely.
