The role supports the Home Care Coordinator with care-related tasks, validates insurance, and manages referrals and communications to ensure timely initiation of home care services.

Qualified candidates have 1–2 years of data entry and customer service experience in health care, strong organizational skills, and EPIC training.

Qualifications

• High school diploma or GED required.
• 1–2 years data entry and customer service in health care, including payor verification and authorization.
• Excellent interpersonal and organizational skills; strong written and verbal communication.

Responsibilities

• Assist the Home Care Coordinator with functions related to home care products and community resources.
• Verify insurance and provide information to agency and patient records.
• Review referral packets to ensure all information is present.
• Communicate with agency to initiate care and confirm referrals.
• Ensure patient discharge occurs or follow up with delays or changes.
• Maintain positive relationships with unit staff and outside agencies.
• Input HCC information into flow sheets accurately and timely.
• Assist with scheduling Homecare 101s for assigned HCCs.
• File logs and clinical documentation; complete time studies on time.
• Ensure requested services are arranged and alert on delays.
• Maintain updated master resource manual and inventory for the office.
• Respond to all internal and external requests promptly.
