Job summary

Johns Hopkins Care at Home in Baltimore is seeking a Home Care Coordinator/Discharge Planner to coordinate patient transitions from hospital to home; this role does not provide hands-on clinical care.

The position requires a BSN, active RN licensure and CPR, with at least two years of clinical nursing experience; home care or discharge planning experience is preferred.

Day shift, 40 hours per week, based at Johns Hopkins Health System, with a comprehensive benefits package and free parking.

Qualifications

The Home Care Coordinator Discharge Planner specializes in coordinating home care services and resources for patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care.

Work Location: JHH

Weekly Working Hours: Days, 40 hours per week

Job Requirements

• Bachelor of Science in Nursing (BSN) required
• Licensure in clinical discipline (RN)
• Active CPR
• Two (2) years experience in clinical nursing required, previous home care or home care discharge planning experience preferred
• Knowledge of Medicare Conditions of Participation preferred
• Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services preferred.

Salary Range:

Minimum 39.64/hour - Maximum 61.45/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
