Job summary

Guardian Angel Home Care seeks a Speech-Language Pathologist to join our nationwide team delivering in-home therapy. This part-time/PRN role focuses on evaluating and treating speech, language, cognition, voice, and swallowing disorders in patients in their homes.

You will collaborate with physicians, nurses, therapists, patients and families to create individualized treatment plans, document progress, and ensure compliance with regulations.

Qualifications

• Master’s Degree in Speech-Language Pathology from an accredited program.
• Current Speech-Language Pathologist license in the state of practice.
• CCC-SLP preferred.
• Current CPR certification (or willingness to obtain).
• Valid driver’s license, reliable transportation, and automobile insurance.
• Strong communication, organizational, and time management skills.
• Home health experience preferred, but not required.

Responsibilities

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services in accordance with professional standards and agency policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers regarding treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Complete timely and accurate clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain compliance with all federal, state, and agency regulations.
