Job summary

The Integrated Care Clinical Manager - Massachusetts at CareSource/Commonwealth Care Alliance (CCA) is a community-based registered nurse responsible for monitoring, follow-up and clinical care management to dually-eligible enrollees with complex medical, behavioral and social care needs. This role focuses on coordinating enrollee care to improve health outcomes and quality of life.

Qualifications

• AS degree in nursing required.
• Active MA RN license required.
• Experience in care coordination or case management preferred.
• Medicaid/Medicare managed care experience preferred.

• Lead the interdisciplinary care team to develop holistic care plans.
