Job summary

CareSource, in partnership with Commonwealth Care Alliance (CCA), seeks an Integrated Care Clinical Manager in Massachusetts to oversee complex care management for dually-eligible enrollees. The role emphasizes home-based assessments, care plan development, and coordination with providers and community resources across Medicare and Medicaid.

Qualifications

• Associates of Science degree in nursing from an accredited program.
• Licensed RN with the ability to independently serve people with complex needs.
• Case management Certification is highly preferred.

Responsibilities

• Engage enrollees in home and community settings to establish a complex care management relationship.
• Coordinate care transitions between providers and community resources.
• Conduct comprehensive and functional assessments and risk assessments.
• Involve enrollees in care plan development and updates as status changes.
• Lead the interdisciplinary care team to create holistic, medically and socially focused plans.
• Monitor long-term services utilization and ensure access to desired living arrangements.
• Assist members in accessing housing, transportation, food assistance, and social services.
• Educate members on benefits, medications, preventive care, and self-management.
