Job summary

Powerback Rehabilitation seeks a dedicated Physical Therapist Assistant to provide in-home outpatient rehabilitation for older adults. You will operate under the supervision of a Licensed Physical Therapist in diverse home settings, including private residences and independent living facilities.

The role offers a flexible schedule across various locations with emphasis on patient-centered care, functional independence, and safe, effective therapy delivery in the home environment.

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver's license or reliable transportation to/from patients' residence.

Responsibilities

• Attends and contributes to patient care, staffing conferences and other required meetings as a representative of physical therapy under the direction of the physical therapist.
• Adheres to department and patient treatment schedules and documentation requirements.
• Collaborates with the rehabilitation team to implement programs for each patient.
• Acts as an assistant to the Physical Therapist during tests, evaluations, and complex procedures.
• Instructs patients and caregivers in the PT treatment program under supervision.
• Maintains timely documentation for all patients treated.
• Cleans and maintains equipment and reports repairs as needed.
• Delivers patient-centered care to improve function and independence.
• Provides specific PT treatments including therapeutic exercise, functional training, manual therapy and other modalities.
• Performs other related duties as required.
