Job summary

Optimal Care - Evergreen seeks a physician to provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to deliver thoughtful, patient-centered care.

You will lead medical care, supervise NPs and PAs, coordinate with families, and improve patient outcomes while maintaining a manageable patient census. Travel within Jackson, MI area is required. Monday–Friday, 8–5, with flexible scheduling possible.

Qualifications

• Doctor of Medicine or Osteopathy required.
• Current unrestricted State license to practice medicine required.
• Board certification in family practice, internal medicine, geriatric or palliative care required.
• Reliable transportation, valid driver’s license, and valid automobile insurance required.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in patients' homes or care settings.
• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Coordinate care with patients, families, and interdisciplinary teams; provide medical leadership and mentorship to Nurse Practitioners and Physician Assistants.

Education
