Job summary

Optimal Care is seeking Nurse Practitioners or Physician Assistants to deliver high-quality, patient-centered care to geriatric and medically complex patients, often in their homes or care settings. You will drive comprehensive evaluations, craft treatment plans, prescribe medications, and coordinate with physicians and caregivers in a collaborative practice.

The role requires daily travel within Toledo and surrounding areas, a Monday–Friday schedule, and a base salary of $110,000–$125,000 USD

Qualifications

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation and valid driver's license and insurance.

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients, often in homes or care settings.
• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Prescribe and monitor medications and coordinate with physicians and caregivers.
• Serve as a clinical decision-maker within a collaborative practice framework.
