Job summary

Qualifications

• Graduate of an approved nursing program with RN licensure.
• English proficiency and ability to work in patient homes.
• State license or eligible for licensure and transferability.
• Ability to plan and deliver care in home environments.

Responsibilities

• Provide direct veteran care and care coordination in HBPC.
• Perform case management, triage, education, and resource coordination.
• Plan and deliver competent care in veterans' homes with complex needs.
• Collaborate on policy and guideline development; evaluate outcomes.
