Responsibilities

• Serve as the primary care provider for a defined patient panel in homes and communities.
• Diagnose and manage chronic and acute conditions with MD leadership and support.
• Address social determinants of health such as housing and nutrition.
• Conduct wound care, post-hospital transitions, and condition-specific assessments.
• Complete comprehensive evaluations including medication reconciliation and fall risk.
• Use Bloom’s data ecosystem to track hospitalizations and post-discharge visits.
• Collaborate with care teams to ensure continuity and reduce readmissions.
