Bloom Healthcare is hiring a Primary Care NP/PA to deliver patient-centered primary care in home and community settings within the Dallas area. You will work with an interdisciplinary team to manage complex cases, focusing on autonomy, relationship-based care, and meaningful outcomes.

Qualifications

• Master's degree in Nursing (MSN) or higher is required.
• Active NP/PA licensure (Missouri licensure referenced in ad) and board certification preferred.
• Experience delivering home-based primary care is a plus.

Responsibilities

• Provide longitudinal, holistic primary care in patients' homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with support from the care team.
• Address social determinants of health and coordinate post-discharge needs.
