Job summary

Bloom Healthcare in Lone Tree, CO is seeking a Primary Care NP/PA to deliver relationship-based primary care in patients' homes and communities with strong team support.

You will manage a defined patient panel, address chronic and acute needs, and leverage Bloom’s data ecosystem to improve health outcomes for homebound seniors.

Compensation includes a $100,000–$125,000 salary with comprehensive benefits starting Day 1 and generous PTO and CME options.

Qualifications

• Master’s degree and Colorado licensure required.
• NP or PA with ability to practice in Colorado.
• Experience in home-based and senior care is preferred.

Responsibilities

• Serve as primary care provider for defined patient panel in homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with team support.
• Address social determinants of health including housing, nutrition, caregiver support, and access barriers.
• Conduct wound care, post-hospital transitional care, and condition-specific assessments.
• Complete comprehensive evaluations including medication reconciliation, fall risk, advanced care planning, and patient education.
• Leverage Bloom’s data ecosystem to track hospitalizations and coordinate timely post-discharge visits (72 hours).
• Collaborate with internal and external care teams to ensure continuity and reduce readmissions.
