Job summary

Bloom Healthcare in Monument, CO seeks a NP/PA to provide relationship-based primary care to homebound seniors in homes and communities. You’ll practice independently with strong support from an interdisciplinary team and data systems to improve health outcomes.

You’ll enjoy a manageable schedule: 8–12 patients per day, Monday–Friday, light call, weekends off, and comprehensive benefits starting day one, including health/dental/vision, PTO, 401(k), and CME allowances.

Qualifications

• Master’s degree in NP or PA and Colorado licensure are required.
• Passion for serving seniors and medically complex populations.
• Strong communication and trust-building skills for home-based care.
• Motivation by mission, relationship-driven care, and meaningful outcomes.

Responsibilities

• Serve as the primary care provider for a defined patient panel in homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with team support.
• Address social determinants of health and barriers to care.
• Conduct wound care, post-hospital transitions, and condition-specific assessments.
• Perform comprehensive evaluations including med reconciliation and fall risk assessments.
• Use Bloom’s data system to track outcomes and coordinate post-discharge visits.
