Job summary

Bloom Healthcare is seeking a Primary Care NP/PA to deliver relationship-based primary care to homebound seniors in Greeley, CO and surrounding communities. You will provide holistic medical care in patients' homes and connect with an interdisciplinary team and data systems to improve outcomes.

The ideal candidate holds a Master’s in NP or PA studies and an active Colorado license, with a passion for seniors and complex care.

Qualifications

• Master's degree in nursing or physician assistant studies and active Colorado licensure.
• Experience delivering primary care in home or community settings is preferred.
• Strong communication, relationship-building, and independence in practice.

Responsibilities

• Provide longitudinal, relationship-based primary care for homebound patients in homes and communities.
• Diagnose, manage, and treat acute and chronic conditions with support from an interdisciplinary team.
• Address social determinants of health and coordinate with care teams to prevent hospitalizations.
• Conduct comprehensive evaluations, planning, and health education for patients and caregivers.
• Utilize data systems to monitor outcomes and coordinate timely post-discharge visits when needed.
• Collaborate with internal and external care teams to ensure continuity of care.
