Job summary

Abode Care Partners is seeking a compassionate Nurse Practitioner to join our Norfolk home-based primary care team. You will conduct clinical assessments, develop personalized care plans, and manage chronic conditions with an emphasis on preventative health in patients’ homes.

Qualifications

• Active state license as NP/PA and current CPR certification.
• MSN required for Nurse Practitioners; PA pathway possible without MSN.
• Maintains DEA licensure and in-good-standing licensure.
• 5+ years nursing healthcare experience; 2+ years NP/PA preferred.

Responsibilities

• Provide primary care in home settings for seniors and complex patients.
• Perform clinical assessments and develop personalized care plans.
• Treat acute visits, manage chronic conditions, and coordinate transitions.
• Order/interpret labs and tests; collaborate with interdisciplinary teams.
• Maintain up-to-date medical records and ensure HIPAA compliance.
• Guide referrals and support preventive health initiatives.
