Job summary

Abode Care Partners seeks a compassionate Nurse Practitioner to deliver primary care to seniors and patients with complex needs in patients' homes. You will conduct clinical assessments and build personalized care plans across skilled nursing, assisted living, and independent living communities.

Qualifications

• Active state license as NP/PA.
• MSN required for NP; PA may not require master's degree.
• Current CPR and DEA licensure.
• Two years of NP/PA experience preferred.
• Strong communication and time-management skills.
• Willingness to provide home-based care in SNF/AL/IL settings.

Responsibilities

• Performs evidence-based medical care by planning, directing, and evaluating patient medical needs.
• Examines and treats chronic and urgent illnesses, minor injuries and other conditions.
• Orders and interprets laboratory and diagnostic testing as needed.
• Collaborates with caregivers and other healthcare professionals to maintain a comprehensive plan of care.
• Provides referrals to other healthcare practitioners when necessary.
• Keeps patient medical records up to date.
