Qualifications

• Active state NP license with national certification.
• MSN required for Nurse Practitioners.
• Current CPR and DEA licensure.
• 5+ years nursing healthcare experience; 2+ years NP/PA preferred.
• Strong interpersonal and written communication skills.
• Proficiency with EMR systems and MS Office.
• Driver's license with valid insurance.

Responsibilities

• Provide primary care services to seniors in home settings.
• Conduct clinical assessments and develop care plans.
• Manage chronic conditions and acute visits with prevention focus.
