Job summary
Abode Care Partners, part of BrightSpring Health Services, is seeking a compassionate Nurse Practitioner to deliver personalized primary care to seniors in home settings. Qualifications
- Active state NP license and current NP board certification.
- Active state DEA licensure.
- MSN degree in nursing.
- Five or more years of nursing experience; NP experience preferred.
- Strong interpersonal and time management skills; proficient with EMR.
- Knowledge of HIPAA and OSHA regulations.
Responsibilities
- Provides primary care services to seniors and patients with complex needs in home settings.
- Performs clinical assessments and develops personalized care plans.
- Collaborates with interdisciplinary teams to ensure seamless care transitions.
- Manages chronic conditions and acute visits with a focus on preventative care.
- Maintains accurate and up-to-date medical records.
