Job summary

Powerback Rehabilitation seeks a licensed Physical Therapist to provide in-home outpatient rehabilitation for older adults, including evaluation, program development, and caregiver education. You will work in patients’ homes or in independent living facilities, delivering customized therapy plans to restore function and independence.

This role offers a flexible schedule across various settings; the team supports professional development, CEUs, and visa/relocation options for eligible candidates.

Qualifications

• Must hold a valid Physical Therapy license in practice state(s).
• Master’s degree in Physical Therapy or equivalent with required licensure.
• Bachelor’s degree in Physical Therapy plus 5 years of PT experience or equivalent.
• Foreign PT degree must be evaluated for US equivalence.
• Valid driver’s license or reliable transportation to patients’ homes.

Responsibilities

• Provide in-home outpatient rehabilitation for older adults.
• Assess needs, develop and deliver PT programs in home or facility settings.
• Educate patients/caregivers to promote independence and safety.
• Coordinate referrals to other disciplines as needed.
• Maintain timely documentation and adhere to schedules.
