• Valid Physical Therapy license in the state of practice.
• Master’s degree in Physical Therapy (or PT degree with equivalent)
• Driver’s license or reliable transportation to patients' homes.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Develop and implement individualized PT plans of care.
• Educate patients and caregivers on exercises and safety at home.
• Document treatment and progress in patient records.
• Delegate to PT assistants per licensure and supervise as required.
