Qualifications

• Experience in in-home/outpatient rehab is preferred.

Responsibilities

• Provide outpatient rehabilitation services in the in-home setting for older adults.
• Assess needs, develop and deliver physical therapy programs to facilitate rehabilitation.
• Supervise Physical Therapy Assistants and students as applicable.
• Maintain timely clinical and billing documentation and attend related meetings.
• Educate patients and caregivers to promote independence and functional improvement.
