Residential Home Health and Hospice is seeking a Nurse Practitioner to provide in-home palliative care in the Plymouth area. This role emphasizes compassionate symptom management, patient and family partnerships, and autonomy within an interdisciplinary team.

You will conduct comprehensive assessments, manage complex symptoms, and plan individualized care while ensuring thorough EMR documentation and adherence to care goals.

Competenze

• BSN from an accredited School of Nursing.
• MSN from an accredited School of Nursing.
• Minimum of five (5) years of clinical experience.
• Hold, in good standing, a license as RN and NP in the practicing state.
• Community-based Home Health or Hospice/Palliative Care experience preferred.

Mansioni

• Deliver compassionate, patient-centered palliative care in patients' homes.
• Perform comprehensive assessments and manage symptoms of serious illness.
• Develop individualized treatment plans addressing physical, emotional, and psychosocial needs.
• Facilitate goals-of-care discussions and educate patients and caregivers.
• Coordinate care with the interdisciplinary team and maintain compliant EMR documentation.
