Gentiva seeks a compassionate Palliative Care Nurse Practitioner to deliver advanced home-based palliative care, guiding patients and families through treatment with an interdisciplinary team.

You will assess, plan, implement, and adjust care, prescribe medications as allowed, and maintain compliant documentation while coordinating with physicians and care teams. This is a full-time home health role with call coverage as needed.

Qualifications

• Master of Science in Nursing from an accredited school; state law applies.
• Minimum two years of Nurse Practitioner experience; family or adult medicine preferred.
• Experience with Part B billing and autonomous practice settings.
• Familiar with Medicare/Medicaid regulations and interprofessional teams.
• Proficiency with electronic health records and point‑of‑care tools.

Responsabilités

• Collaborate with the Palliative Medical Director and care team to deliver comprehensive, patient-centered palliative care.
• Perform in-depth patient assessments, including physical, emotional, and psychosocial evaluations.
• Develop and implement individualized care plans in partnership with patients and families.
• Manage and adjust treatment plans based on patient needs and progress.
• Prescribe medications (including controlled substances) and order diagnostic tests as permitted by state law.
• Maintain timely, accurate clinical documentation in compliance with professional standards.
• Communicate effectively with physicians, caregivers, and other healthcare professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services when appropriate.
