Job summary

Senior Care Partners PACE is seeking an Occupational Therapist to join our interdisciplinary team and help participants live independently at home and in the community. You will assess, plan, and provide customized therapy to improve ADLs and IADLs while collaborating with physicians, nurses, and other therapists.

Qualifications

• Doctoral degree in Occupational Therapy required.
• Active Michigan OT license required.
• At least 1 year experience with frail, elderly, or long-term care populations.
• ACOTE accreditation or equivalent required.
• Ability to collaborate effectively with an IDT.

Responsibilities

• Assess and develop personalized care plans for participants in their homes.
• Provide occupational therapy to improve or maintain ADL/IADL function.
• Educate participants and families on safety, adaptations, and strategies.
• Collaborate with IDT and supervise COTAs and staff.
• Maintain accurate documentation and participate inQuality Improvement.
