Job summary

The Occupational Therapist (OT) at the PACE Program center will assess and treat participants to maximize independence and safety in home and community settings. You will work with an interdisciplinary team to tailor care plans to each participant’s goals and conditions.

Responsibilities include conducting assessments, developing treatment plans, providing OT interventions in multiple settings, and advising on adaptive equipment while ensuring regulatory compliance and accurate documentation.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Licensed Occupational Therapist in Indiana.
• Current CPR certification.
• Valid driver's license and reliable transportation for home visits.
• At least 1 year of experience with geriatric population.

Responsibilities

• Conduct initial and ongoing OT assessments to evaluate functional abilities and rehabilitation potential.
• Develop individualized treatment plans based on evaluation results and participant goals.
• Provide direct occupational therapy interventions in the center, home, or community settings.
• Recommend and assist with adaptive equipment and home modifications.
• Train participants and caregivers in ADL strategies and device use.
• Document evaluations, progress, and discharge summaries per standards.
• Participate in IDT meetings to coordinate care planning.
• Collaborate with therapists and nurses to support participant safety.
