Job summary

Senior Care Partners PACE is seeking an Occupational Therapist to join our interdisciplinary team to help participants live independently and safely in their homes and communities. You will assess, plan, and implement OT services tailored to each participant’s environment, goals, and daily routines.

You’ll collaborate with physicians, nurses, social workers, and COTAs to deliver person-centered care, modify environments, and promote meaningful participation in daily activities.

Qualifications

• Doctoral degree in Occupational Therapy or grandfathered via previous OT degree.
• ACOTE®-accredited OT program or equivalent.
• Current, active Michigan OT license.

Responsibilities

• Conduct comprehensive initial and periodic OT assessments in participants' homes.
• Develop participant-centered plans addressing home care and functional needs.
• Evaluate need for OT and home care services and communicate with the IDT.
