Job summary

HarmonyCares seeks a Nurse Practitioner to deliver in-home primary care to high-acuity patients in a patient-centered manner. You will focus on meaningful, personalized care rather than volume and dedicate time to address each patient’s needs in their home setting.

This full-time role is Monday–Friday with support in select markets; a company car with driver may be available and a competitive benefits package accompanies the position.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA license/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification.
• Valid driver's license with good driving record.

Responsibilities

• Practice medicine with a focus on patient care, not volume, taking the time needed to address patient needs.
• Provide primary care to high-acuity patients in their home, Monday through Friday.
• Collaborate with care teams to coordinate services and treatment plans.
