Job summary

HarmonyCares is seeking a Nurse Practitioner to deliver high-quality in-home primary care to complex patients. The role emphasizes patient-centered care and collaborative teamwork, with duties Monday through Friday.

The successful candidate will hold NP licensure in the practicing state, possess a DEA license or eligibility, and have CPR/BLS certification. Geriatrics experience is preferred and a valid driver's license is required.

Qualifications

• Licensed NP in the state of practice.
• Federal DEA license/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to high acuity patients in the home, Monday–Friday.
• Deliver patient-centered care with focus on quality rather than volume.
• Collaborate with care teams to coordinate in-home services.
