Job summary

HarmonyCares is seeking a Nurse Practitioner to provide in-home primary care for high-acuity patients in the Washington region. This role emphasizes patient-centered care over volume, with a Monday–Friday schedule and support staff in select markets.

Qualified candidates are licensed NPs with DEA eligibility, CPR/BLS certification, and board certification. Experience in geriatrics and home health is preferred; valid driver’s license and good driving record required.

Qualifications

• Licensed as an NP in the state of practice.
• Federal DEA license or eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification in a relevant specialty.

Responsibilities

• Provide primary care to high-acuity patients in their home.
• Take time to address patient needs, focusing on quality care over volume.
• Support appointments with an assistant in select markets; Monday–Friday schedule.
