Job summary

HarmonyCares is a leading national value-based provider of in-home primary care services for people with complex healthcare needs. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume, will address the patient’s needs directly in patients’ homes.

The Nurse Practitioner will provide primary care to high-acuity patients in their home, Monday through Friday, with additional support as needed and a focus on high-quality, compassionate care.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• The Nurse Practitioners we seek have directly related experience in home health, geriatrics, hospice and primary care.
• We want clinicians to take time to truly address the patient’s needs and practice medicine with a focus on patient care, not volume.
• Provide primary care to high acuity patients in their home, Monday - Friday.
