Qualifications

• Licensed NP in the state of practice.
• Active Federal DEA license or eligibility.
• Geriatric training/experience preferred.
• CPR/BLS certification current.
• Board certification in nurse practitioner specialty.
• Maintain a valid driver's license and good driving record.

Responsibilities

• Provide direct in-home primary care to high-acuity patients in their homes.
• Collaborate with an integrated care team to coordinate patient care.
