Job summary

HarmonyCares seeks a Nurse Practitioner to deliver in-home primary care to high-acuity patients in select markets. The candidate will practice medicine with a patient-centered approach, prioritizing quality care over volume.

Ideal applicants have NP licensure, DEA eligibility, CPR/BLS, and board certification in family or geriatrics, plus geriatric/home health experience. A valid driver’s license and vehicle access are required.

Qualifications

• Licensed NP in the state of practice.
• DEA license/eligibility required.
• Geriatric/home health experience preferred.
• Active CPR/BLS certification.
• Board Certification required.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide in-home primary care to high-acuity patients.
