Optimal Care in Jackson, MI is seeking a Nurse Practitioner or Physician Assistant to provide primary and consultative care to geriatric and medically complex patients, often in homes or care settings, with autonomy to focus on high-quality, patient-centered care.

You will conduct evaluations, manage treatment plans, prescribe medications, and coordinate with physicians, caregivers, and a multidisciplinary team to improve comfort, function, and quality of life.

Qualifications

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation, valid driver’s license, and valid automobile insurance.

Responsibilities

• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Prescribe and monitor medications and coordinate with physicians and caregivers.
• Serve as a key clinical decision-maker within a collaborative practice framework.
• Help patients and families understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations.
