Optimal Care in Bingham Farms, MI is looking for a Nurse Practitioner or Physician Assistant to provide primary and consultative care to geriatric and medically complex patients. This role supports meaningful patient relationships with no weekends or on-call obligations.

The candidate will conduct evaluations, manage treatment plans, and collaborate with a supportive clinical team. Benefits include a flexible schedule, competitive salary of $110,000—$120,000, and generous paid time off among others.

Qualifications

• Minimum 2 years clinical experience in a post‑acute setting.
• Adult, Family, or Gerontology certification preferred.
• Experience with geriatric patients.

Responsibilities

• Conduct comprehensive medical evaluations.
• Develop and manage individualized treatment plans.
• Prescribe and monitor medications.
• Coordinate closely with physicians and caregivers.

Skills

Education
