Job summary

Optimal Care in Michigan seeks a Nurse Practitioner or Physician Assistant to deliver primary and consultative care to geriatric and medically complex patients, often in homes or care facilities, with autonomy to focus on high-quality, patient-centered care.

You will conduct evaluations, develop treatment plans, prescribe medications, and coordinate with physicians and the interdisciplinary team to avoid unnecessary hospitalizations and improve quality of life.

Qualifications

• Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.

Responsibilities

• Perform comprehensive medical evaluations and develop treatment plans.
• Prescribe and monitor medications; coordinate with physicians and team.
• Provide primary and consultative geriatric care in home or facility settings.
• Help patients understand diagnoses and reduce hospitalizations.
