Job summary

Optimal Care - Evergreen seeks a Nurse Practitioner or Physician Assistant to deliver primary and consultative care to geriatric and medically complex patients, often in patients' homes or care settings, with strong autonomy to focus on high-quality and patient-centered outcomes.

You will perform evaluations, craft treatment plans, prescribe medications, and coordinate with physicians and the care team within a collaborative practice.

Qualifications

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients.
• Conduct comprehensive medical evaluations and develop treatment plans.
• Prescribe and monitor medications and coordinate with physicians and care team.
• Serve as a clinical decision-maker within a collaborative practice framework.
• Help patients and families understand diagnoses and manage chronic illness.
