SouthernCare is seeking a compassionate Hospice RN / RN Case Manager to guide patients and families through the hospice journey, delivering skilled nursing care at home and coordinating with an interdisciplinary team.

You will assess patient needs, manage plans of care, educate families, and ensure timely documentation while upholding quality, compliance, and compassionate support in end-of-life care across Indiana communities.

Qualifications

• RN with nursing licensure in good standing
• Minimum 1 year of RN experience in hospice, home health, or related settings
• Experience collaborating with physicians, social workers, and care teams preferred
• Ability to educate patients and families on disease, care plans, and end-of-life needs

Responsabilités

• Perform comprehensive patient assessments and reassessments in home or facility settings
• Deliver skilled nursing interventions and manage symptoms per plan of care
• Develop and update individualized care plans including visit frequency
• Coordinate care with IDGs and physicians to optimize outcomes
• Document clinical findings, orders, and progress notes timely
• Provide patient and caregiver education on illness, care, and medications
