Compassus in Indiana is seeking a Hospice Registered Nurse (RN) Case Manager to coordinate care for patients at home and in facility settings. You’ll evaluate needs, develop care plans, and collaborate with a multidisciplinary team to deliver compassionate, person-centered care.

Ideal candidates have an active RN license in the state, two years of nursing experience (hospice preferred), and CPR/BCLS certification.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse; hospice experience preferred.
• Current driver's license.
• Current CPR and BCLS certification.
