Job summary

Elara Caring is seeking a Hospice Nurse Practitioner to join our team. You will provide comprehensive hospice and palliative care, perform admissions and transfers, and work with physicians and an interdisciplinary team to deliver care in patients’ homes.

The role requires an NP license, a Master’s in nursing, and NP certification, with at least 1 year of clinical nursing experience and hospice/palliative care exposure preferred.

Qualifications

• Current Nurse Practitioner license in the practicing state.
• Master’s degree in nursing and national NP certification.
• 1+ year of clinical nursing experience.
• Maintain active DEA certification if state allows controlled substances.
• Ability to frequently lift up to 50 pounds and assist patient transfers.
• Valid driver’s license or reliable transportation to patient homes.
• 1+ year hospice/palliative care post-graduate experience preferred.

Responsibilities

• Perform routine and emergency hospice visits, including admissions and transfers.
• Assess patient needs, document findings, and coordinate with physicians and care team.
• Provide bedside palliative care and support to patients and families.
• Triage patient needs to prevent avoidable hospitalizations.
• Maintain accurate patient records and CMS-compliant documentation.
• Collaborate with Medical Director and interdisciplinary team to ensure high-quality care.
• Provide training and education to clinical staff on hospice/palliative care.
